
Form Manager makes it easy to create practically any online database form you
may need. Each form can include up to 25 questions and up to 17 response
choices per question.
A simple interface walks you through choosing your field types then typing in
your question and field descriptors. After you have added all the questions you
need, you can choose how you want your data distributed (via email, database,
or both). Another feature enables you to customize the "Thank You" for your
response page, so important invoices or other call to action instructions can
be created. For even more design flexibility, ConnectBuilder's easy-to use
editor is positioned at the top and bottom of each form so you can add
additional text and images as needed. A give-away prize picture and information
on how to enter your contest is just one example.
With other CB Tools:

When setting up an email mailing list in List Manager, when you choose the
"Opt-in Web Form" option, a Subscribe Form and a Thank You page are
automatically created for you. The Subscribe Form and Thank You page can then
be viewed and changed in Form Manager.

With Website Manager, you can easily add Form Manager's custom and opt-in forms
to your CB website. You can also choose to add images and text for your forms
via your web page rather than to the form itself.
Standalone: When using Form Manager as a standalone tool, all your forms are
made available online via URL links.
Soon to be released features: The ability to perform purchase transactions with
Form Manager forms will be an add-on feature and available in the near future.
This feature will step you through setting up credit card fields on your forms,
and will ensure that all transactions are secured.
